- From the Amplify Websites dashboard, navigate to Communications > FMS Forms.
- Click Create a New Form. From the right side of your screen, choose one of the following 3 ways to build a form:
- Choose a template from the list of pre-made templates.
- Click Start from Scratch for a blank slate.
- Click Copy Existing Form to create a new form that is similar.
- Enter a name for the form into the Form Name field. *If you copied an existing form, the Form Name field will already be populated, but feel free to change it!
- Click Build Form. This will load a new form into the form builder where the Form’s properties can be adjusted, new fields added, adjust existing fields, and manage payment options (if combined with Amplify Giving).
